Agreements

As someone attending HTBH playgroup I am aware of, and agree to the following:
Term fees are a $25 donation per term, or $40 for more than one child in the same family
I agree to follow the recommended exclusion periods for infectious diseases as provide to me in the 'Staying Healthy in Childcare Chart.' I also agree to notify the coordinator if we fall ill with any infectious disease.
I am aware of the recommended sun safety and that it is my responsibility to provide a hat, sun cream and appropriate clothing to prevent over exposure to the sun.
I am aware that the supervision and safety of the children I bring to playgroup is my responsibility
I am aware that management of my child's behaviour is my responsibility, and that any behaviour which continually endangers the safey of other children and/or carers may result in the forfeiture of my position
I am happy for photos to be taken (by both Playgroup coordinator and/or other carers) that may include my child in a playgroup setting
I will not post any image of another child at playgroup on any social media accounts, without parental consent (see below for exclusion)
I am aware of the expectation that everyone who attends will participate in creating a positive environment as well as helping to organise, clean up, or otherwise help to the best of their abilities.

In filling and submitting this registration form I indicate my willingness to participate with my child in the playgroup activities at Holy Trinity Anglican Church Baulkham Hills. There may be photographs taken of the activities by the Playgroup Coordinator, these may include me and.or my child, and may be used for promotional purposes only.
I understand that whilst all reasonable care will be taken, I accept full responsibility for myself and my child.
